Testimonial Terms and Conditions

I understand and agree that my testimonial (the “Testimonial”), provided for the benefit of MedPro Group Inc. and its direct and indirect subsidiaries (collectively, “MedPro”), may be used in connection with publicizing and promoting MedPro. I authorize MedPro to use the Testimonial in whole or in part, and to format the Testimonial to fit the piece or medium.

I authorize MedPro to copy, exhibit, publish or distribute the Testimonial for purposes of publicizing MedPro or for any other lawful purpose. The Testimonial may be used in printed publications, multimedia presentations, on websites or in any other distribution media. I agree that I will make no monetary or other claim against MedPro for the use of the Testimonial and I waive any right to inspect or approve the finished product, including written copy, wherein my likeness or my Testimonial appears. I release MedPro from all claims, demands and causes of action which I or any other persons acting on my behalf may have by reason of this authorization or otherwise related to the Testimonial.

I agree that my Testimonial can be attributed using my specialty and name as I have selected it to be attributed on the form.
